Case description: A 28-years old patient was admitted to emergency room with symptoms of acute abdomen. He also has a bloody stool. Besides he has gonorrhoea, which was recognized 2 days ago.

 

Questions : What was the reason? What's the treatment?

It isn't general inflammatory state caused by gonorrhoea. Swab from anus correct, in swab from urethra cultured Neisseria gonorrhoeae.

parameters of clotting normal, in ultrasound nothing special. Before all procedure, pacient first of all should undergo an operation.

The reason of embolus was Nicolai syndrome. Benzathini benzylpenicilli num is crystalline penicilin and crystals precipitated and cause block of superior mesenteric artery - therefore ischemic colitis and symptoms of acute abdomen.
